H1N1 Vaccine Provider Information

Central Vaccine Recovery Program

The U.S. federal government has developed the voluntary "Central Vaccine Recovery Program" to collect all unused doses of 2009 H1N1 influenza vaccine. Providers with an assigned PIN may take advantage of this voluntary program to return any unused monovalent 2009 H1N1 flu vaccine doses remaining in their practice.

• As in the initial phases of the Recovery Program, providers with an assigned PIN will receive a shipping label late September/October to return unused 2009 H1N1 doses. Doses must be retuned in a sealed and appropriately packaged container. Ancillary supplies CANNOT be returned.
• If you do not receive a shipping labels, need to locate your clinic's PIN, or for assistance with your return questions, please call the Health and Human Services Supply Center at 1-800-642-0263 (4:00 am - 4:00 pm PST)

• Packing Slip
  You can complete this packing slip electronically. Note that "Manufacturers" and "Formulation" columns are menus with pre-entered choices, use scrolling arrows to the right of the column to choose manufacturer and formulation. You can type in the first letter of the manufacturer or formulation and the entire name will appear. Click on the field to confirm your choice. When done, simply print out a copy to be placed in the box.

2009 Monovalent H1N1 Influenza Vaccine

The Centers for Disease Control and Prevention (CDC) and the California Department of Public Health has discontinued distribution of the 2009 monovalent H1N1 flu vaccine, as of July 30th 2010. Calpanflu is no longer available for ordering.

The shelf life for existing supplies of the 2009 monovalent H1N1 Vaccine has been shortened. All remaining doses of sanofi pasteur multi-dose vials will expire by September 15, 2010, regardless of the date on the label.
